We kindly ask you to update your health history if there has been any significant changes to your health (major surgery, allergies, new medication, etc.).

3. COVID-19 PRE-SCREENING QUestions

To help keep everyone safe and healthy, we have implemented a new safety protocol where all patients have to answer the pre-screening questions and pass 24-hours before their appointment.
